3/14/2023 EnchantaVaughn Approved Behavior: As the Jahsim prepared session. Jahsim was in a calm mood, as evidenced by his quiet and relaxed expressions. Jahsim wore comfortable clothes and displayed good hygiene. Jahsim was observed being a good listener while QMHP reviewed his objectives and goals. QMHP-C engaged mother in a discussion about behaviors since the last session. Mother continues to talk about his behaviors in the school setting and how she really does not know what else to do when he gets in trouble at school. Behavior: The Jahsim shared that he had a good day. Jahsim has implemented interventions and strategies taught by QMHP in his daily activities. Jahsim said that catching up with all of his work and spending more time with his mom has made things more loving in the home. Jahsim was observed conducting himself appropriately without hyperactive behaviors. Jahsim symptoms of ADHD have been decreasing due to avoiding distractions. Jahsim shares his daily experiences in order to reflect during the session. Intervention: QMHP went to the home for the purpose of meeting with each family member for family session and meeting with Jahsim individually to discuss his mental health. QMHP engaged in conversation with each family member regarding Jahsimâs services. QMHP reiterated the expectations and rules for family session. QMHP also monitored effects of medication and coping practices and reported progress to mom. QMHP listened to mother verbalize any issues or concerns she had with his medication. QMHP explored the Jahsimâs capability to cope with his stressors and to get back on track. QMHP assisted Jahsim with finding and utilizing necessary resources at home. QMHP advised Jahsim to make the right choices and to keep up the good work. QMHP processed Jahsim positive behaviors. QMHP inquired about possible challenges Jahsim can experience with applying this skill. QMHP provided positive feedback. QMHP applauded Jahsim for making the big decision to want to work on changing his behavior. Intervention: QMHP motivated Jahsim to respond to exercise and express his feelings. QMHP inquired about Jahsimâs day and what things is he doing to make it a good day. QMHP probed Jahsim about his schedule in efforts to understand his daily routine. QMHP discussed with him the importance of working hard to make sure that he continues to stay focused on his goals. QMHP challenged Jahsim to continue displaying a positive attitude while employing positive reinforcement when Jahsim is able to achieve his goals and objectives. QMHP listened as the Jahsim communicated his feelings with his mom. QMHP supported Jahsim in approaching his mother with his ideas. QMHP encouraged the Jahsim to continue to work on understanding his feelings and making sure that he responds in a positive way. QMHP provided praise when the Jahsim continued to display positive behaviors throughout the day, remaining on task and was very respectful to all adult figures. Response: Jahsim exhibited a positive attitude toward his family. Jahsim smiled and hugged QMHP. The Jahsim responded in a positive manner acknowledging the positive reinforcement that the QMHP provided by saying âthank youâ while presenting a smile. Jahsim utilized relaxation strategies appropriately. The Jahsim showed his gratitude for being able to release physical tension stating that he felt relaxed. Jahsim was able to cooperate and comply to rules and regulations due to positive reinforcement provided by the QMHP. Jahsim was able to utilize coping skills to eliminate stressors and unwanted feelings. Jahsim displayed self-determination to stay focused and pay attention. Jahsim interacted well with others and demonstrated good team building skills. Jahsim displays a good attitude when communicating his thoughts about himself. Jahsim did an excellent job by showing interest in the session by asking appropriate questions. Response: Jahsim communicated that he felt safe and comfortable talking to the in-home counselor. Jahsim informed QMHP that he was coping and utilizing deep breathing techniques to stay calm and not overreact. Jahsim demonstrates problem solving by staying calm and using his senses. Jahsim displays good social behavior, as observed and supported by mental health professional. Jahsim is reported building healthy relationships in the home.
